Abstract

The latest clinical trials of stroke patients undergoing mechanical thrombectomy v/s better medical treatment showed favorable results even in patients with an extensive ischemic core. A recent meta-analysis showed that it was even beneficial for patients with extensive ischemia and up to 24 hours since the last time seen well (Saraj, 2023). In this article, we review the different clinical trials of thrombectomy, the pathophysiology of ischemia and propose the astrocyte and the glymphatic system as the focus on the effects of treatment. We propose the need to change the paradigm of current treatment in patients with extensive ischemic core, as well as the need to consider these changes in the financing and organization of a national thrombectomy network by government authorities.
