This article explains the different tensions towards the Integral Health Model arising from the community diagnoses carried out in the different Primary Health Care health centers in the district of Buin. The information was collected through focus groups with leaders, social leaders linked to the nine health centers of the district, based on the interpretative and critical analysis of the results revealed in the diagnostic instances, it was concluded that the main challenge of the Primary Health Care model in Buin is to broaden the current epistemological framework and orient it towards other approaches that help to better understand the community development and relevance of the diverse territories of the district, challenging the current model, forcing it to broaden its horizons of understanding, and therefore, to reformulate the current limits of what to do in local health policy and planning.
