Prevención del embarazo en mujeres con alto riesgo de aborto. Resultados de un proyecto de intervención en la comunidad

Objective: This is a cohort controlled study with intervention on low socioeconomical community level, to determine the preventive effect of family planning on ilegal clandestine abortion. Method: Three similar communities were compared.One of them was intervened with high quality clinical services and follow up of women with high abortion risk. Another community had, high quality clinical service alone, without follow up of potential contraceptive users. The third community was the control and had the rutinary medical care clinical services. The women of three community were interviewed before the intervention and two years after of the begining of intervention. Both surveys to the same women, were compared on abortion rates, fecundity, regarding use of contraception and abortion risk level. Results: The abortion prevalence had a significative descend in the women with abortion high risk, when was direct and totally intervened. The fertility rates go down in the three communities, but more in the community not intervened. As conclusion: The high quality family planning service on high abortion risk women, in a media where the abortion is totally penalized, has significative impact on its prevention, without affecting the natality rates. It is recommended to apply this methodology on primary health care to prevent the provoked and clandestine abortion.
